Introduction: Diabetes is a prevalent disease with irreparable damages such as renal disease, blindness, terminal extremity necrosis and consequently amputation and death. This study intended to determine the relationship between different risk factors and the incidence of long term compli-cations in patients with type 2 diabetes mellitus. Materials & Methods: Medical records of 208 patients with type 2 diabetes mellitus (73 male and 135 female) admitted to Golestan hospital in Ahwaz city during 1384 were studied. For each patient, variables such as sex, age, triglycerides and cholesterol level, duration of diabetes, family history, body mass index, race and hypertension were gathered and analyzed using the binary logistic regression model in SPSS software, version 17.0. Findings: Our findings revealed that family history of diabetes (P=0.006, OR=3.97) and age (P<0.001, OR=1.18) the patients had a significant relationship with the incidence of complications. Other risk factors did not show any statistical relationship with these complications. Discusion & Conclusin: In general, the obtained results from the present study suggested that people with the family history of type 2 diabetes mellitus should follow health recommendations to prevent the disease and its related complications
