A rare case of preeclampsia associated with acrocyanosis and mild clubbing

Introduction:  Preeclampsia is a severe pregnancy-related disorder characterized by hypertension and multi-organ damage. Along with hemorrhage and infection, it is one of the three leading causes of maternal mortality worldwide. The aim of this study was to report a case of preeclampsia accompanied by acrocyanosis and mild clubbing.
Case Report: In this study, a 32-year-old woman (G3P2) at 36 weeks of gestation presented with complaints of dyspnea, dizziness (particularly when lying in the left lateral position), intermittent nausea and vomiting, morning dryness of the lips and mouth, acrocyanosis, and mild clubbing. Her medical history was notable for two previous episodes of preeclampsia, prior cystocele surgery, previous rhinoplasty, and a weight gain of 12 kg over the preceding 1–2 months. Despite timely and appropriate therapeutic interventions, the patient remained hospitalized in the intensive care unit for 7 days with persistent acrocyanosis that did not improve despite oxygen therapy. She was subsequently discharged and referred to another center for magnetic resonance imaging (MRI).
Conclusion: This case highlights endothelial dysfunction induced by placental factors in preeclampsia, with particular emphasis on peripheral vascular manifestations such as acrocyanosis and clubbing. These findings underscore the importance of recognizing such signs as potential indicators of severe vascular involvement and suggest the need for further studies to evaluate their prevalence, pathophysiology, and prognostic significance in patients with preeclampsia.
