Introduction: Overweight and obesity are associated with metabolic disorders, including changes in adipokines such as vaspin. The effect of exercise training on vaspin is inconsistent. The aim of this systematic review and meta-analysis was to investigate the effect of exercise training on serum vaspin levels and glycemic indices in overweight and obese Iranian women. Materials & Methods: PubMed, Web of Science, Scopus, Magiran, Irandoc, Noor Mags, and SID databases were systematically searched for articles published up to 11 March 2024. The standardized mean difference (SMD) and 95% confidence interval (CI) were calculated using a random model and CMA2 software to determine the effect. The (I2) test was used to determine heterogeneity, and the Funnel Plant test and Egger test were used to examine publication bias. Results: A total of 14 studies on 376 overweight and obese female subjects were meta-analyzed. The results showed that exercise training significantly reduced insulin resistance [SMD = -0.706 (-1.128 to -0.283), P = 0.001] and had a non-significant reduction in vaspin [SMD = -0.312 (-0.628 to 0.004), P = 0.053], glucose [SMD = -0.566 (-1.161 to 0.029), P = 0.062], and insulin [SMD = -0.232 (-0.900 to -0.435), P = 0.495] compared to the control group. Conclusion: The results of the present meta-analysis showed that exercise training significantly improved insulin resistance in obese and overweight Iranian women through changes in serum vaspin and glycemic indices.
