ویرایش ژنوم با سامانه CRISPR-Cas9 در سندرم لینچ: از بینش‌های مولکولی تا چشم‌اندازهای بالینی

مقدمه: سندرم لینچ شایع‌ترین علت سرطان کولورکتال ارثی است و حدود ۳۵٪ موارد سرطان کولورکتال را شامل می‌شود. این سندرم عمدتاً ناشی از جهش‌های ارثی در ژن‌های ترمیم عدم‌تطابق DNA (MMR) شامل MLH1، MSH2، MSH6 و PMS2 بوده و با ناپایداری میکروساتلایت (MSI) و افزایش خطر بروز سرطان‌های متعدد همراه است.
مواد و روش­ها: این مطالعه یک مرور روایی بر شواهد موجود درباره کاربردهای فناوری CRISPR/Cas9 و رویکردهای نوین‌تر شامل Base Editing و Prime Editing در زمینه سندرم لینچ انجام داد.
یافته ­های پژوهش: شواهد نشان می‌دهد فناوری CRISPR می‌تواند برای (۱) ایجاد مدل‌های سلولی و حیوانی دقیق به‌منظور مطالعه مکانیسم‌های بیماری، (۲) توسعه ابزارهای تشخیصی سریع و حساس جهت شناسایی MSI، (۳) اصلاح جهش‌ها در مدل‌های آزمایشگاهی و (۴) طراحی رویکردهای درمانی فردمحور مورد استفاده قرار گیرد. Base Editing و Prime Editing امکان اصلاح دقیق برخی جهش‌های نقطه‌ای را بدون ایجاد شکست دورشته‌ای DNA فراهم می‌کنند.
بحث و نتیجه‌گیری: در مجموع، اگرچه فناوری‌های ویرایش ژنوم پتانسیل قابل‌توجهی برای بهبود تشخیص و درمان سندرم لینچ نشان می‌دهند، فاصله معناداری تا کاربرد بالینی روتین وجود دارد. چالش‌هایی نظیر اثرات خارج‌هدف، محدودیت‌های انتقال به بافت هدف، ناهمگنی ژنتیکی بیماران و ملاحظات اخلاقی و قانونی همچنان موانع اصلی محسوب می‌شوند. رفع این چالش‌ها از طریق بهبود سامانه‌های انتقال، استفاده از ارگانوئیدهای مشتق از بیمار و انجام مطالعات بالینی دقیق، شرط اساسی برای تحقق کاربرد بالینی ایمن و مؤثر این فناوری‌ها است.
