Sci. 1563-4728 2588-3135 doi 2007 14 4 Biological Evaluation of Soya Protein Quality Abstract Introduction: Evaluation of the quality of food proteins have a great importance because of its biological and economical asfects. However biological, microbiological, chemical and combined methods have been used and introduced for determining quality of Proteins. Among the exsisted methods, NPR, RNPR,TPD and PER have been suggested as appropriatie methods for determining quality of Proteins. This study has been conducted to evaluate of quality protein of soya, using the mentioned methods on the soya products in 2003. Materials & Methods: This experimental study has been made on 48 male wistar rats, aged 21-23 days under 6 food diet in boundles of 8 including 3 diet soya, Casein+Methionine and control (free Protein) in order to study RNPR and PER (The condition of studing RNPR and PER are the same with the exception that the duration of study on PER is 28 days without limitation of Protein intake) and 3 diet soya, Casein+Methionine and control for studing TPD. Period of study for NPR was 14 days. Protein intake, weight increase was determined for calculating NPR. Period of study for TPD was 9 days. Nitrogen intake and fecal Nitrogen were determined for calculating TPD. Protein intake and change in weight of animals were determined. Rata of NPR, RNPR, TPD and PER was evaluated by T- test. Results: Indicator of NPR was calculated 3.650.35 and 4.370.48 for soya and Casein+Methionin respectively (P<0.01) and RNPR=83. Indicator of TPD was calculated 83.232.5 and 93.452.22 for soya and Casein+Methionin respectively (P<0.0001). Indicator of PER was 2.280.35 and 3.040.24 for Soya and Casein+Methionin respectively (P<0.001). Conclusion: Soya protein as compared with Casein is of lower quality. : Protein Quality Soya Biology. 2007 1 01 1 6 http://sjimu.medilam.ac.ir/article-1-39-en.pdf

Sci. 1563-4728 2588-3135 doi 2007 14 4 A Study on Efficacy of Pethidine Hydrochloride and Diclofenac Sodium Diclen) on Pain Severity after Cesarean Section ... Abstract introduction: Pain is a post-operation problem in patients. Cesarean section is a common surgery among women and many patients experience pain after surgeries for a reason or another. Although using narcotic drugs for controlling post-operative pain is a wide- spread routine, side effects of such drugs have caused a limitation in their use. Materials & Methods: This is an applied, mono-blind, trial clinical study performed to assess the effectiveness of pethidine hydrochloride and diclofenac sodium on pain relief after cesarean in Shahid Mostafa Khomaini hospital of Ilam (in 2006). During the trial, 100 women undergoing cesarean (50 women in pethidine group and 50 women in diclofenac group) were observed and assessed. While the patient had pain, intra muscular analgesic (in accordance with the group) was injected for her, then she was assessed by Visual Analog Scale, V.A.S, and compared. The data was analyzed by chi- square, Mann-Whitney, ranked- regression and Tstudent tests. Results: According to results, Mann-Whitney test showed that pain severity in diclofenac sodium was lower than that in pethidine group. There was a singnificant decrease of pain, statistically, in the second and third 20 minutes and 120 minutes after the intervention (p<0.001). Discussion: Considering the findings of this research, diclofenac sodium (non-narcotic analgesic) is more effective than pethidine hydrochloride (narcotic analgesic) to relieve the pain after surgical operations amony patients. Sci. 1563-4728 2588-3135 doi 2007 14 4 A Study on Microinjection Effects of Metabotropic Glutamate Receptors on Appetite Abstract Introduction: There is little investigation performed regarding the use of metabotropic glutamate receptors in birds. In this study, we had investigated the use of metabotropic glutamate receptor agonist and antagonist to determine the appetite of broiler cockerels. Material and methods: 12 broiler cockerels of were reared until 700-750 grams were bred up. Each was steriotaxically implanted into the right lateral ventricle. Then, the birds were injected with DHPG(1µM) and AIDA(2mM) and cumulative food intake was monitored at different times post injection with computerized program. The birds were given the mentioned food in a replicated latin square design on the 1st, 3rd, 5th and 7th days in 4 groups. Results: In comparison with the control group, the intracerebroventricular administration of DHPG increased significantly (average 28.7 percent), food intake and AIDA had decreased (average 30 percent) significantly (P<0.05). Conclusion: According to these results it can be concluded that metabotropic glutamate receptors can, significantly, affect on the appetite of broiler cockerels to intake food. : Appetite Metabotropic Glutamate Receptor Broiler Cockerel. 2007 1 01 13 18 http://sjimu.medilam.ac.ir/article-1-46-en.pdf

Abstract Introduction:The introduction of color Doppler Ultrasonography has improved the evaluation of vertebral arteries. This study was designed to determine normal values of hemodynamic parameters in second segment of vertebral artery in healthy old people. Materials & Methods: We performed a prospective study in 31 healthy adults from 60 to 96 years old. Angle corrected flow velocities and luminal diameters were measured and waveform parameters calculated in all vertebral arteries. Results: It was possible to visualize V2 Segment in 100% and origin of right VA in 72% and origin of left VA in 59% of them. Mean diameter of right VA was 32.4± 0.56 mm while it was 3.52± 0.67 mm for the left VA. The resistance index in right VA was 0.71±0.08 and it was 0.69±0.06 in the left VA.There was no significant differences between the two sides. Peak systolic velocity was 35.77±12.71 cm/s on the right and 36.29±12.71 cm/s on the left. Accordingly, no differences were observed between the two sides. Conclusion: This study and its presented reference values can lead to better differentiation between normal and pathologic findings in vertebral arteries among old people. Nevertheless, further studies are needed to determine the normal and pathologic values of vertebral arteries. : Vertebral Artery Doppler Sonography Peak Systolic Velocity Resistance Index 2007 1 01 20 23 http://sjimu.medilam.ac.ir/article-1-51-en.pdf

Journal of Ilam University of Medical Sciences J. Ilam Uni. Med. Sci. 1563-4728 2588-3135 doi 2007 14 4 Prodution of Polyclonal Antibody Against Giardia Lamblia in Rabbit ... Abstract Introduction: Giardia lamblia is the most frequently human intestinal protozoan in the worldwide. Diagnosis of G. lamblia by microscopic examination of stool is common. ELISA; IFA methods have researching and epidemiological use. For designing a powerful test with high detection rate like dipstick or rapid diagnosis kits first step is production of polyclonal antibody. For this reason we decided to produce polyclonal antibody against G. lamblia in rabbit. Material and methods: Giardia cysts were purified from human fecal samples by sucrose two stage modified gradient method. Their concentration was independently adjusted to 1× 106/ml PBS, were inoculated intramasculary-intradermaly in four stage with freunds complete adjuvant for first time and with freunds uncomplete adjuvant for second time and without freunds adjuvant for next time, in rabbit. After immunization of rabbit, IFA test was carried out to evaluate the polyclonal antibody production against Giardia. Results: The results of test showed the rabbit was immuned. Best results of IFA were achieved with 1×106/ml parasite and sera dilution of 1/100 and goat anti rabbit IgG conjugated with FITC dilution of 1/20 is under immunofloursent microscope. Conclusion: Giardias antigens have an important role for diagnosis in clinical and other samples. Production of anti Giardia polyclonal antibody with high titer is first and important step for designing IFA and direct ELISA kits for direct detection of Giardias antigens. Produced polyclonal antibody can use purification and conjugation with enzyme. Production Giardia lamblia Polyclonal antibody 2007 1 01 25 30 http://sjimu.medilam.ac.ir/article-1-53-en.pdf

Sci. 1563-4728 2588-3135 doi 2007 14 4 The Effect of Intravenous Lidocaine Premedication and Slow Rate of Fentanyl Administration on Fentanyl – Induced Coughing ... Abstract Introduction: Intravenous (IV) administration of Fentanyl bolus elicits cough in more than one – third of the patients. Coughs may sometimes be explosive leading to hypoxia, vomiting, with increases in intracranial, intraocular and intraabdominal pressures. Materials & Methods: This double blind clinical trial included 360 patients below 60 years with ASA status I & II, who were scheduled for elective surgeries. They were randomized into 3 groups of 120. Group I received Normal Saline as placebo, 60 secounds late fentanyl was administred slowly over a period of 30 secounds. Group II was given fentanyl over 5 secounds, 1 minute after Lidocaine, (1.5 mg/kg IV). Group III also received Normal Salin, 60 secounds before the Fentanyl was given in 5 secounds. Fentanyl dose was 2 µg/kg. Then, incidence and severity of cough was recorded, and the results were analyzed using Chi square test. P.value of 0.05 was considered significant. Results: The incidence of cough was 2.5% in group I, 9.2% and 22.5% in groups II & III respectively. The finds showed that the incidence of cough was significantly higher in group III (P<0.001). Occurrence of cough in patients under 18 was more than those above 18 years of age in all the three groups (P=0.001). Conclusion: IV Lidocaine and, in particular, slow rate of Fentanyl administration, significantly minimize Fentanyl-Induced coughing. Also the incidence of evoked cough is much higher in young patients. : Fentanyl Cough Lidocaine Injection time 2007 1 01 31 35 http://sjimu.medilam.ac.ir/article-1-55-en.pdf

Journal of Ilam University of Medical Sciences J. Ilam Uni. Med. Sci. 1563-4728 2588-3135 doi 2007 14 4 Electrochemical study of Activated HpB binding to Human Serum Albumin ... Abstract Introduction: Human serum Albumin (HSA) is a single strand protein that plays a major role in the maintenance of plasma oncotic pressure. Surface of this protein is covered with unneutralized negative charges and has an important role in the transfer of some drugs , fatty acids , cations and hormones in the blood . Materials & Methods: HAS (Hexadesyl pridinium bromide ) was purchased from Sigma company and the other reagents were recieved from Merck Co. UsingPhosphate buffer in pH=7 for all experiments through CD- spectrophotometer (model Jasco) secondary structure changes in the protein were observed. The ligand binding of HSA- HPB interaction was studied by fluke potentiaometer. HSA concentration in all experiments was 0.2 mg/ml. Results: HSA CD spectra in the far – UV region were obtained at 370C and 420C . The binding isotherms and also scadchard curves for HAS-HPB interaction at 370C and 420C were drawn and discussed . Conclusion: Rising temperature from 370C up to 420C proved a considerable effect on the structural aspects of HSA. HSA- HPB interaction depends on the temperature so the affinity of HSA for HPB binding reduces as temperature increases to 420C from 370C . Therefore, the amounts of negative charges on the surface of HSA at the 420C are less then those at 370C and leads to the decrement of blood oncotic pressure. This effect expresses the considerable effect of HSA on the moderation of fever on the body. Abstract Introduction: Pseudohypoparathyroidism is an uncommon disorder related to Calcium and phosphorous metabolism with peripheral resistance to PTH. There are many clinical types of such disorders. Pseudohypoparathyroidism with maniacal disorder has just been reported only once ina 13-year-old girl. Materials & Methods: This case was a 16- year- old boy who presented an elevated mood along with aggression, irritability, visual hallucination, paranoid and grandoise delusion. Thus, he was admitted in a mental with diagnosis of bipolar disorder. Except for a 1.5 hours history of dizziness, disorientation and aggression, The patient complained of no other physical signs. Then, he underwent a treatment with 900mg lithium for one month. Accordingly, the disorder symptoms reduced but a complete remission was not achieved after ward. Having done more typical investigations, Pseudohypoparathyroidism was diagnosed and confirmed with laboratory tests such as Calcium and phosphorous and PTH and alkaline Phosphatas levels e Conclusion: After taking two tablets of Rocatrol and two tablets of calcium as well as lithium. The patient regained his complete recovery. He, then, followed up the treatment for 6 years and has showed no signs of hyperactivity, impulsivity or emotional disorders from which he had been suffering since childhood.
