fa برنامه ثبت لیشمانیوز جلدی (سالک) در استان ایلام ( خلاصه سیاستی) A strategic summary of Cutaneous leishmaniasis registration program in Ilam province انگل شناسی پزشکی medical parasitology كاربردي Applicable <div style="text-align: justify;">مقدمه: لیشمانیازیس به عنوان یکی از شش بیماری عفونی مورد تأکید سازمان جهانی بهداشت است. شهرستان های مهران و دهلران در جنوب استان ایلام از کانون های آندمیک لیشمانیازیس جلدی (سالک) در کشور هستند. استان ایلام  با دارا بودن 420 کیلومتر مرز مشترک با کشور عراق در معرض بسیاری از بیماری های زئونوز و گرمسیری از جمله بیماری سالک قرار دارد. لذا پیشگیری، تشخیص اولیه بیماری و درمان از ضروریات می باشد. برنامه ثبت یا (Registry) یک جزء جدا ناپذیر و ضروری سیستم های جامع مراقبت بیماران بوده که از آن می توان به عنوان ابزاری برای نشان دادن سیر بیماری استفاده نمود. این برنامه با تامین داده های لازم به عنوان ابزار مفیدی برای استفاده مدیران در برنامه های مراقبت می باشد. در رجیستری علاوه بر داده های بیماری کلیه اطلاعات مربوط به ریسک فاکتورهای مهم و پیامد بیماران (Follow up) نیز ثبت و جمع آوری می شود. مواد و روش ها : برنامه ثبت لیشمانیوز جلدی (سالک) در مناطق اندمیک استان ایلام با هدف مقایسه روش های صحیح تشخیص این بیماری با روش رایج تشخیص (میکروسکوپی) و تعیین میزان خطاهای انجام شده توسط کارشناسان آزمایشگاه در قالب یک مطالعه مقطعی از اردیبهشت 1398 لغایت شهریور 1400 می باشد. یافته های پژوهش: با استفاده از روش های مولکولی (PCR) از مجموع 10 درصد کل لام های مثبت دریافتی از دو شهرستان دهلران و مهران حدود 5 درصد مثبت کاذب و از کل لام های منفی ارسالی ، 28% منفی کاذب تشخیص داده شد. لازم به ذکر اینکه زمان اجرای برنامه ثبت سالک در استان ایلام همزمان با اپیدمی بیماری کرونا بود. بحث و نتیجه گیری:  با توجه به یافته های این مطالعه و اهمیت بیماری به دلیل آندمیک بودن آن در بخش جنوبی استان ایلام، وجود آمار صعودی میزان شیوع بیماری سالک در سال های گذشته، همزمانی تردد زائرین اربعین حسینی با دوره انتقال بیماری، عوارض روحی و جسمی ناشی از بیماری و مهم تر از همه اطمینان از گزارش های ارجاعی کارشناسان مراکز بهداشتی و آزمایشگاهی در خصوص تشخیص عامل بیماری سالک ، نحوه تشخیص و گزارش دهی و حذف جواب های مشکوک، مثبت و منفی کاذب ، پیشنهاد می گردد تا نسبت به راه انداری مرکز تشخیص جامع مولکولی بیماری در استان های کشور  اقدام تا بتوان بیماران را با تشخیص صحیح تا مرحله بهبودی و کامل کردن روند درمان مورد پیگیری قرار داد.  </div> <pre class="tw-data-text tw-text-large tw-ta" data-placeholder="ترجمه" data-ved="2ahUKEwib9PX1zOOBAxVgdqQEHXLdCXYQ3ewLegQICRAO" dir="ltr" id="tw-target-text" style="text-align: justify;"> Introduction: Leishmaniasis is one of the six infectious diseases emphasized by the World Health Organization. Mehran and Dehloran cities in the south of Ilam province are endemic centers of cutaneous leishmaniosis in the country. Ilam Province, having near to 420 km common border with Iraq, is exposed to many zoonotic and tropical diseases, including Cutaneous leishmaniasis. Therefore, prevention, early diagnosis and treatment are essential. The registration program is an inseparable and necessary component of comprehensive patient care systems, which can be used as a tool to show the status of the disease. By providing sufficient data, this program has a useful role for managers to use in health care planning. Registry is a necessary component of comprehensive patient care systems, which can be used as a tool to show the status of the disease, which is called follow-up. Material & Methods: The aim of Cutaneous leishmaniasis registration program in the endemic areas of Ilam province is collection and comparing the correct methods of diagnosis disease with the common one (microscopy) and determining the number of errors made by laboratory experts in the form of a cross-sectional study from May 2018 to august It is 2020. Findings: By using molecular tests (PCR) to 10% of all positive slides received from Dehloran and Mehran cities, about 5% were false positives, and 28% of all negative slides collected was false negative. It should be mentioned that the implementation of the registration program in Ilam province during the epidemic of corona disease.
