بررسی وضعیت بیماران با عفونت تنفسی و مداخلات پیشگیریکننده از مصرف آنتیبیوتیک در این بیماران

<span style="font-size:8pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:"Times New Roman",serif"><b><span lang="FA" style="font-size:9.0pt"><span style="font-family:"B Zar""><span style="color:black"><span style="letter-spacing:.1pt">مقدمه:</span></span></span></span></b> <span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">استفادۀ بیشازحد و تجویز نامناسب آنتیبیوتیکها برای عفونتهای حاد دستگاه تنفسی </span></span>ARIs<span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">)) یکی از عوامل اصلی مشکلات فعلی مقاومت آنتیبیوتیکی است. عفونتهای حاد دستگاه تنفسی (</span></span>ARIs<span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">) جزو شایعترین عفونتها هستند و نظر به انتشار بسیار گستردۀ آنها، اهمیت بالایی دارند و میتوانند به مرگومیر کودکان و بزرگسالان منجر شوند؛ بنابراین، هدف از این مطالعه آموزش مهارتهای ارتباطی پزشک با بیماران برای کاهش مصرف آنتیبیوتیک، کاهش مقاومت آنتیبیوتیکی و استفادۀ مناسب از آنتیبیوتیکها در مواردی که مصرف آن ضروری است و نیز کاهش خوددرمانی و درنهایت، کاهش هزینهها است.</span></span></span></span></span></span><br>
<span style="font-size:8pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:"Times New Roman",serif"><b><span lang="FA" style="font-size:9.0pt"><span style="font-family:"B Zar""><span style="color:black"><span style="letter-spacing:.1pt">مواد و روش­ها:</span></span></span></span></b> <span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">پژوهش حاضر، مطالعهای مقطعی دربارۀ مدیریت مصرف آنتیبیوتیک از سوی بیماران مبتلا به عفونت تنفسی مراجعهکننده به پزشک است.</span></span></span></span></span></span><br>
<span style="font-size:8pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:"Times New Roman",serif"><b><span lang="FA" style="font-size:9.0pt"><span style="font-family:"B Zar""><span style="color:black"><span style="letter-spacing:-.1pt">یافتههای پژوهش:</span></span></span></span></b> <span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">نتایج این مطالعه نشان داد که آنتیبیوتیک کو-آموکسیکلاو بیشترین تجویز و سفکسیم کمترین تجویزی آنتیبیوتیکی بودهاند که بهترتیب متخصصان کودکان و متخصصان گوارش و کبد تجویز کردهاند؛ همچنین نتایج این مطالعه نشان داد که آنتیبیوتیک سفکسیم که پزشکان تجویز نمودهاند، از لحاظ آماری معنیدار بود (</span></span>P< 0.05<span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">).</span></span></span></span></span></span><br>
<span style="font-size:8pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span style="font-family:"Times New Roman",serif"><b><span lang="FA" style="font-size:9.0pt"><span style="font-family:"B Zar""><span style="color:black"><span style="letter-spacing:-.1pt">بحث و نتیجهگیری</span></span></span></span></b><span lang="AR-SA" style="font-size:9.0pt"><span style="font-family:"B Zar"">: خوشبختانه در میان پزشکان عمومی آنتیبیوتیک آموکسیسیلین و سفکسیم و در میان متخصصان کودکان آموکسیسیلین، سفکسیم و آزیترومایسین و متخصصان بیماریهای عفونی کو-آموکسیکلاو کاهش تجویز در نسخ داشته است؛ اما متأسفانه برای متخصصان گوارش و کبد افزایش تجویز نسخ آنتیبیوتیکی گزارششده است و این امر میتواند به افزایش مقاومتها و بهتبع آن، شکست درمان منجر گردد؛ بنابراین، راهکاری که بتواند باعث شکل گرفتن دستورالعملی اجرایی برای تجویز منطقی آنتیبیوتیکها و بهتبع آن کنترل عفونتها، کاهش مقاومتهای آنتیبیوتیکی، کاهش مدتزمان بستری بیماران و نیز میزان مرگومیر ناشی از عفونت و کاهش هزینههای درمان برای بیمار و بیمارستان گردد، ضروری است..</span></span></span></span></span></span><br>
<span style="font-size:9pt"><span style="font-family:"Times New Roman",serif"><b><span style="color:black"><span style="letter-spacing:-.2pt">Introduction:</span></span></b> Overuse and inappropriate antibiotic prescription for acute respiratory infections (ARIs) are among the major contributors to the current antibiotic resistance problem. ARIs are among the most common infections, and due to their high prevalence, they are of great importance and can play a vital role in mortality among children and adults. Therefore, this study aimed to train physicians in terms of communication skills with patients to reduce the use of antibiotic, which results in minimizing antibiotic resistance, proper use of antibiotics when it is necessary to use, and minimize self-medication, which ultimately leads to reducing costs.</span></span><br>
<span style="font-size:9pt"><span style="font-family:"Times New Roman",serif"><b><span style="color:black"><span style="letter-spacing:-.2pt">Material & Methods:</span></span></b> This cross-sectional study was conducted to control antibiotic consumption in patients with respiratory infections referring to physicians.</span></span><br>
<span style="font-size:9pt"><span style="font-family:"Times New Roman",serif">(Ethic Code: IR.Medilam.Rec.1395.112)</span></span><br>
<span style="font-size:9pt"><span style="font-family:"Times New Roman",serif"><b><span style="color:black"><span style="letter-spacing:-.2pt">Findings:</span></span></b> The results of this study showed that cefixime and co-amoxiclav antibiotics were the least and the most prescribed by gastroenterologists and hepatologists, as well as pediatricians, respectively. In addition, according to the results, cefixime antibiotic showed statistically significant results (P<0.05).</span></span><br>
<span style="font-size:9pt"><span style="font-family:"Times New Roman",serif"><b><span style="color:black"><span style="letter-spacing:-.2pt">Discussion & </span></span></b><b><span style="font-size:10.0pt"><span style="letter-spacing:-.2pt">Conclusion:</span></span></b> The results revealed a reduction in the prescriptions of cefixime and amoxicillin by general physicians, amoxicillin, cefixime, and azithromycin by pediatricians, and co-amoxiclav by infectious disease specialists. However, there was an increase in antibiotic prescriptions by gastroenterologists and hepatologists which could lead to resistance increase, and consequently, the treatment failure. Accordingly, it is of necessary importance to take measures and implement instructions for logical prescription of antibiotics leading to infection control, reducing antibiotic resistance, reducing the length of patients’ hospital stay, as well as mortality due to infection, and reducing treatment costs for patients and hospitals.</span></span><br>
