<strong><em><span style="font-family:b mitra;"><span style="font-size:10.0pt;">مقدمه: </span></span></em></strong><span style="font-family:b mitra;"><span style="font-size:10.0pt;">اختلال تعادل و آسیب های ناشی از آن در افراد سالمند و بیماران با اختلالات نرولوژیک شایع می باشد. ارتوزهای پا-مچ پایی می توانند با بهبود اختلالات حرکتی بر تعادل تاثیر بگذارند. بنا بر این هدف از مطالعه حاضر بررسی تاثیر ارتوز پا-مچ پایی اصلاح یافته بر تعادل افراد سالمند سالم بود.</span></span><br>
<strong><em><span style="font-family:b mitra;"><span style="font-size:10.0pt;"> مواد و روش ها:</span></span></em></strong> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">10 فرد سالمند سالم بالای 60 سال</span></span> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">در این مطالعه شرکت کردند. در این مطالعه از ارتوز پا-مچ پایی مرسوم و ارتوز اصلاح شده پا-مچ پایی که قسمت خلفی ارتوز در ناحیه پاشنه جهت آزادسازی قسمت خلفی پا و مفصل ساب تالار برداشته شده بود، استفاده شد.</span></span> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">شاخص های تعادل با دستگاه صفحه نیرو در وضعیت ایستادن روی دو پا با چشمان باز و بسته و طی سه وضعیت مختلف(ارتوز پا-مچ پایی مرسوم، ارتوز پا-مچ پایی اصلاح شده و بدون ارتوز) اندازه گیری شد. هم چنین تعادل عملکردی با استفاده از آزمون برخاستن و رفتن زمان دار بررسی شد.</span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:10.0pt;"></span></span></span><br>
<strong><em><span style="font-family:b mitra;"><span style="font-size:10.0pt;">یافته های پژوهش:</span></span></em></strong> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">نتایج این مطالعه نشان داد که استفاده از ارتوز پا-مچ پایی اصلاح شده در مقایسه با حالت پا برهنه منجر به تغییرات معناداری در متغیرهای تعادل ایستایی نمی شود(</span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:10.0pt;">P>0.05</span></span></span><span style="font-family:b mitra;"><span style="font-size:10.0pt;">). در حالی که پوشیدن ارتوز پا-مچ پایی مرسوم منجر به کاهش معنی دار(16 درصد) جا به جایی مرکز فشار در راستای داخلی-خارجی در مقایسه با حالت پابرهنه شد(</span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:10.0pt;">P=0.02</span></span></span><span style="font-family:b mitra;"><span style="font-size:10.0pt;">). <a name="_Hlk525502954"></a></span></span><br>
<strong><em><span style="font-family:b mitra;"><span style="font-size:10.0pt;">بحث و نتیجه گیری:</span></span></em></strong> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">انجام یک اصلاح ساده در ارتوز پا-مچ پایی مرسوم منجر به عدم اختلال در تعادل ایستایی افراد سالمند سالم و هم چنین بهبود عملکرد تعادلی در مقایسه با ارتوز مرسوم گردید. به نظر می</span></span> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">رسد این امر به خاطر افزایش فیدبک های حسی و استفاده از حرکات مفصل ساب تالار باشد. بررسی تاثیر این ارتوز در بیماران نورولوژیک دارای اختلالات حرکتی پا و مچ پا توصیه می</span></span> <span style="font-family:b mitra;"><span style="font-size:10.0pt;">شود.</span></span>
<em>Introduction:</em> Balance impairment and related injuries are common in elderly and patients with neurologic disorders. Ankle foot orthosies can influence on balance via improvement of movement disorders. Therefore the aim of this study was to investigate the effects of modified ankle-foot orthosis on balance performance in healthy elderly.<br>
<br>
<em>Materials & Methods:</em> The research was conducted on ten healthy elderly aged above 60. The conventional ankle-foot orthosis and modified ankle-foot orthosis were used in this study (the posterior part of the orthosis in the heel region was removed to provide free movements in the subtalar joint). Balance parameters were measured with a force plate during double limb standing in opened eyes and closed eyes under three different conditions: conventional ankle foot orthosies, modified ankle foot orthosies and barefoot. Also functional balance was assessed using Timed Up and Go test. Ethics code: IR.MUI.REC.1396.3.345.<br>
<br>
<em>Findings:</em> The results of this study indicated that using modified ankle-foot orthosis did not significantly change standing balance parameters compared to barefoot condition (P>0.05). However, wearing conventional ankle-foot orthosis resulted in a significant reduction of the center of pressure excursion in the mediolateral direction (16%) compared to the barefoot condition (P=0.02).<br>
<br>
<em>Discussion & Conclusions:</em> A simple modification in conventional ankle-foot orthosis did not impair standing balance performance of healthy elderly people. Additionally, the modified ankle-foot orthosis improved balance abilities compared to conventional ankle-foot orthosis. These observations could be attributed to the increased sensory feedback and using subtalar movements. It is recommended to study the effects of this modified orthosies on balance in the patients with neurological impairments who have foot and ankle movement disorders.
