fa بررسی تطبیقی تأثیر کتاب کالیفرنیا و ارزش نسبی خدمات بر هزینه‌های بستری اعمال شایع در بیمارستان‌های منتخب شهر ایلام كاربردي Applicable <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;text-indent:1.3pt;line-height:normal;direction:rtl;unicode-bidi:embed;"><strong><em><span style="font-family:b mitra;">مقدمه:</span></em></strong> <span style="font-family:b mitra;">نظام تعرفه‌گذاری پزشکی ایران که برای اولین بار در سال 1362 با بهره‌گیری از کتاب کالیفرنیا به اجرا درآمد پس از 30 سال به دلایل متعدد متحول شد و با تدوین و ابلاغ کتاب ارزش نسبی خدمات سلامت وارد مرحله جدیدی شد. اجرای کتاب جدید بر هزینه‌های بستری بیمه‌شدگان تأثیر خواهد گذاشت.</span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;text-indent:1.3pt;line-height:normal;direction:rtl;unicode-bidi:embed;"><strong><em><span style="font-family:b mitra;">مواد و روش‌ها:</span></em></strong><span style="font-family:b mitra;"> جامعه آماری مطالعه، پرونده‌های بستری بیمه‌شدگان سازمان بیمه سلامت در بیمارستان‌ها منتخب شهر ایلام در شهریور و مهر 1393 و شایعترین اعمال نمونه‌‌های مطالعه بودند که به روش سرشماری تعیین، داده‌های آنها مطابق اهداف مطالعه در چک لیست ثبت، کدگذاری و وارد رایانه گردید با استفاده از نرم‌افزار </span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:9.0pt;">SPSS21</span></span></span><span style="font-family:b mitra;"> تجزیه و تحلیل انجام شد.</span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;text-indent:1.3pt;line-height:normal;direction:rtl;unicode-bidi:embed;"><strong><em><span style="font-family:b mitra;">یافته‌های پژوهش:</span></em></strong><span style="font-family:b mitra;"> در مجموع، میانگین هزینه اعمال جراحی شایع 95/61% رشد داشت. در شهریور 7% و در مهر ماه 5%&nbsp; هزینه‌ها را بیمه‌شدگان پرداخت نموده‌اند. میانگین سهم سازمان در مجموع 74/76% افزایش یافت. سهم سازمان در مهر با 9% رشد به 87% رسید. هدفمندی یارانه‌ها 11% در شهریور و 5% در مهر را پوشش داد. سهم بیماران در شهریور 9% و در مهر 4% کل هزینه‌ها بود. 1% هزینه‌ها در شهریور و 2% در مهر خارج از تعهد بوده که توسط بیمار پرداخت شده است. بیمه‌های تکمیلی سهم بسیار ناچیز در حد 0% را از هزینه‌ها تقبل نموده‌اند.</span></p> <p dir="RTL" style="margin-bottom:0in;margin-bottom:.0001pt;text-align:justify;text-indent:1.3pt;line-height:normal;text-autospace:none;direction:rtl;unicode-bidi:embed;"><strong><em><span style="font-family:b mitra;">بحث و نتیجه‌گیری: </span></em></strong><span style="font-family:b mitra;">با اجرای کتاب ارزش نسبی و ابلاغ مقررات مربوط به آن سهم نهایی پرداخت ازجیب بیمه‌شدگان از هزینه‌های درمانی با افزایش سهم سازمان و کمک یارانه‌ها به 5% کاهش پیدا کرده است.</span></p> <p style="text-align:justify;text-justify:kashida;text-kashida:0%;text-indent:1.3pt;line-height:normal;"></p> <p><em>Introduction</em><strong>:</strong> &nbsp;The Iran medical tariff rate system that for the first time was carried out enjoying the California book in 1983, after 30 years had been evolved due to numerous reasons and with compilation and prophecy of relative value book of health services entered to the new stage. The implementation of a new book will affect on the hospitalization expenses of insured persons.</p> <p></p> <p><em>Materials & methods</em><strong>:</strong> The population of the study, records of hospitalizedinsuredhealthinsurance organization in the hospitals of the city of Ilam in September and October2014 and were the most common examples of actions that census methods, their data in accordance with the objectives of the studying the Czech records, coded and entered intothecomputerusingSPSS21 softwareanalysiswas performed.</p> <p></p> <p><em>Findings</em><strong>:</strong> Totally, the average cost of common surgical procedures95/61percent. In August 7% and &nbsp;in October 5% of the insured paid their costs; hence, averageshareofa total of74/76% increased. Shareof9percent to87percentinOctober. Respectively, 11% and 5% were covered by subsidy in September and October. 9% of patients in SeptemberandOctoberwas4% ofthe total costs. 1% and2% in October-off costs in September was a commitment paid by the patient.
