بررسی و شناسایی ژن های مقاومت به تتراسایکلین در سویه های اشریشیاکلی انتروپاتوژنیک جدا شده از نمونه های اسهال کودکان به روش Multiplex PCR و تعیین مقاومت آنتیبیوتیکی

<p dir="RTL"><strong><em>مقدمه:</em></strong> برخی از سویه ­های پاتوژنیک اشریشیاکلی می ­توانند باعث ایجاد بیماری های روده­ ای و خارج روده ­ای شوند. اشریشیاکلی انتروپاتوژنیک مهم ترین عامل اسهال کودکان کشورهای در حال توسعه است. هدف از این مطالعه، تعیین میزان شیوع مقاومت آنتی بیوتیکی و توزیع فراوانی ژن ­های مقاومت به تتراسیکلین در اشریشیاکلی مولد اسهال می ­باشد<span dir="LTR">.</span></p>
<p dir="RTL"><strong><em>مواد و روش ­ها:</em></strong> تعداد 150 نمونه مدفوع اسهالی کودکان زیر 5 سال از مراکز درمانی مختلف در شهر تهران که به صورت تصادفی انتخاب شده بودند جمع­ آوری و در نهایت 55 نمونه اشریشیاکلی با آزمون­ های مختلف میکروبی و بیوشیمیایی شناسایی و تایید گردید. آزمون­ حساسیت آنتی­بیوتیکی به روش دیسک دیفیوژن بر اساس دستورالعمل <span dir="LTR">CLSI</span> با آنتی بیوتیک ­هایی از گروه­ های مختلف انجام شد. جهت شناسایی ژن ­های مقاومت به تتراسیکلین از آزمون <span dir="LTR">PCR</span> چندگانه­ ای استفاده شد.</p>
<p dir="RTL"><strong><em>یافته­ های پژوهش:</em></strong> بیشترین مقاومت به آنتی بیوتیک ها، آموکسی­ سیلین و تتراسایکلین به ترتیب 3/36 درصد و 5/27 درصد و کمترین غلظت ممانعت از رشد(<span dir="LTR">MIC</span>) در بین سویه های مقاوم به تتراسایکلین، 6/66 درصد سویه ­ها <span dir="LTR">g/ml</span>µ 128 و 3/33 درصد سویه ­ها <span dir="LTR">g/ml</span>µ 256 گزارش گردید. بیشترین فراوانی ژن­ های مقاوم به تتراسایکلین، ژن <span dir="LTR">tetA</span> و <span dir="LTR">tetB</span> بوده که هر یک به میزان 6 نمونه(20 درصد) شناسایی شدند. ژن <span dir="LTR">tetC</span> در 2 ایزوله و ژن <span dir="LTR">tetD</span> در یک ایزوله شناسایی شدند.</p>
<p dir="RTL"><strong><em>بحث و نتیجه­ گیری:</em></strong> مقاومت آنتی­ بیوتیکی اشریشیاکلی در کودکان مبتلا به اسهال می­ تواند نشان ­دهنده مصرف بی ­رویه آنتی ­بیوتیک ­ها، گسترش کاست های ژنی مقاومت به آنتی بیوتیک و انتقال ژنتیکی در بین جمعیت ها باشد. جهت درمان قطعی و عدم بروز مقاومت در سویه های پاتوژن، تعیین الگوی مقاومتی و میزان <span dir="LTR">MIC</span> جهت پیگیری روند مقاومت ضروری است. </p>

<p><em>Introduction</em>: Some pathogenic strains of Escherichia coli can cause intestinal and external intestinal diseases. Enteropathogenic Escherichia coli is one of the most important factors of children diarrhea in developing countries. The aim of this study was to determine the prevalence of antibiotic resistance and the prevalence of tetracycline resistance genes in Escherichia coli causing diarrhea.</p>
<p></p>
<p><em>Materials </em><em>&</em><em> methods</em>: A total of 150 stool samples of diarrheal children under 5 years from treatment centers in Tehran hospital were selected randomly, collected and finally 55 cases were confirmed as Escherichia coli with different biological and biochemical tests. Antibiogram testing was performed by disk diffusion method according to CLSI guidelines with various antibiotics. Multiplex PCR test was used for identifying the tetracycline resistance genes.</p>

<p><em>Findings</em>: The results showed that the most resistance to antibiotics were amoxicillin and tetracycline 36.3% and 27.5%, respectively, and were reported the minimum inhibitory concentration (MIC) of the strains resistant to tetracycline, 66.6% of the strains 128 μg/ ml and 33.3 % of the strains 256 μg/ ml. The most prevalence were identified of tetracycline resistant genes, genes tetBand tetA in 6 samples (20%). TetC gene identified in 2 isolated and tetD gene in 1 isolation.</p>
<p></p>
<p><em>Discussion & Conclusions</em>: Antibiotic-resistant Escherichia coli in children with diarrhea can indicate excessive use of antibiotics, the spread of antibiotic resistance gene cassettes and genetic transmission among the population. Treatment and absence of resistance gene in pathogenic strains, is necessary determine the resistance pattern and the MIC to follow up the process of resistance.</p>
