fa مقایسه تاثیر بیهوشی عمومی و اپیدورال بر شاخص ‌های تنفسی حین و پس از انجام عمل جراحی کوله سیستکتومی لاپاروسکوپیک متخصص داخلی Internal operations( nursing ) كاربردي Applicable <p dir="RTL" style="margin-right:1.3pt;"><strong><em>مقدمه:</em></strong> عمل جراحی کوله سیستکتومی لاپاروسکوپیک(<span dir="LTR">LC</span>) با استفاده از بیهوشی عمومی از شایع ترین اعمال جراحی محسوب می شود. با توجه به مخاطرات و اثرات بیهوشی عمومی بر شاخص‌ های تنفسی و در نظر گرفتن بیهوشی اپیدورال به عنوان یک جایگزین مناسب و ایمن برای بیهوشی عمومی، این مطالعه با هدف مقایسه شاخص‌ های تنفسی بیماران حین و پس از <span dir="LTR">LC</span> تحت دو روش بیهوشی عمومی و اپیدورال انجام شده است.</p> <p dir="RTL" style="margin-right:1.3pt;"><strong><em>مواد و روش ها:</em></strong> این مطالعه کارآزمایی بالینی تصادفی(<span dir="LTR">equivalent study</span>) به صورت دو گروهی بر روی بیماران کاندید کوله سیستکتومی لاپاروسکوپیک که یک گروه با کمک بیهوشی عمومی و گروه دوم به روش بیهوشی اپیدورال تحت عمل جراحی قرار گرفتند، انجام و شاخص ‌های تنفسی دو گروه قبل، حین و پس از عمل اندازه گیری و مقایسه شدند. تجزیه و تحلیل یافته ها با استفاده از نرم افزار آماری <span dir="LTR">vol.20</span> <span dir="LTR">SPSS</span> انجام گردید.</p> <p dir="RTL" style="margin-right:1.3pt;"><strong><em>یافته‌ های پژوهش:</em></strong> در حین عمل جراحی شاخص های <span dir="LTR">&nbsp;SPO2, PaO2, PaCO2</span>در گروه بیهوشی عمومی کمتر از گروه اپیدورال بود<span dir="LTR"> .</span>بر اساس آنالیز واریانس اندازه های تکراری با کنترل اثر<span dir="LTR">SPO2 </span>&nbsp;&nbsp;پایه و جنسیت روند تغییرات در حین و پس از عمل بین دو گروه معنادار بود<span dir="LTR">(P<0.005)</span>. در گروه تحت بیهوشی عمومی در حین عمل افت<span dir="LTR"> SPO2 </span>بیشتر بود(<span dir="LTR">P<0.05</span>) ولی در پایان عمل جراحی اختلاف معناداری وجود نداشت(<span dir="LTR">P= 0.07</span>).</p> <p dir="RTL" style="margin-right:1.3pt;"><strong><em>بحث و نتیجه گیری:</em></strong> استفاده از روش اپیدورال یا بیهوشی عمومی تغییرات اندکی بر اندکس های تنفسی حین عمل جراحی دارد ولی پس از عمل جراحی تفاوتی در نتیجه استفاده از هر کدام از این روش ها دیده نمی شود و بر حسب ترجیح بیمار و پزشک هر یک را می توان به کار برد.</p> <p dir="RTL"></p> <p><em>Introduction</em>: Laparoscopic cholecys-tectomy with general anesthesia is very common. According to the risks of general anesthesia and its effects on respiratory indices, epidural anesthesia as a safe substitute is used instead of general anesthesia in laparoscopic cholecystectomy patients. This study has been conducted to compare respiratory effects of these two methods of anesthesia on respiratory indices in laparoscopic cholecystectomy patients.</p> <p></p> <p><em>Materials & methods</em>: This study is randomized clinical trial (equivalent study) that has been performed in two groups of laparoscopic cholecystectomy patients anesthesized either by general anesthesia method or epidural method. In both groups respiratory indices were measured before, during and after the surgery and then compared and analyzed using SPSS version 20.</p> <p></p> <p><em>Findings</em>: PaO2, SpO2 and PaCO2were lower in general anesthesia group than the epidural group (P<0.005) intraop-eratively. However after surgery there were no differences between the two groups (P=0.07).
