بررسی مقاومت دارویی سویه های E.coli جدا شده از عفونت های ادراری در شهر ایلام به روش دیسک دیفیوژن و پروفایل پلاسمیدی آن ها
A Study of Antibiotic Resistance Pattern and Plasmid Profile of Uropathogenic Escherichia coli Isolated in Ilam(western Iran)

مقدمه: بررسی الگوی مقاومت آنتی بیوتیک و پروفایل پلاسمیدی صد نمونه ایزوله اشرشیاکلی جدا شـــــده از عفونت های ادراری در شهر ایلام هدف این مطالعه است. مواد و روش ها: ایــزوله ها با روش های مرسوم تشخیص داده شد. تست حساسیت آنتی بیوتیک به وسیله روش دیسک- دیفیوژن (کربی بائر)براساس معیارهای CLSI انجام گرفت. استخراج پلاسمید با استفاده از کیت انجام گرفت. یافته های پژوهش: همه ایزوله ها به مروپنم حساس بودند. میزان مقاومت به آنتی بیوتیک های مختلف به ترتیب ذیل بود: آموکسی سیلین 81 درصد، تتراسایکلین 64 درصد، سیپروفلوکساسین 21 درصد سفپیم و سفتاریدیم 16 درصد، سفتریاکسون، کوتریمکسازول و توبرامایسین 15 درصد، جنتامیسین 12 درصد، نیتروفورانتوئین 5 درصد و آمیکاسین 1 درصد. مطالعات پروفایل پلاسمیدی نشان داد که ایزوله های E.coli حاوی پلاسمیدهایی با اندازه هایی از 5/1 تا21 کیلو دالتون حمل بودند. 88 درصد ایزوله های E.coli دارای یک پلاسمید عمومی با وزن مولکولی 21 کیلو دالتون بودند که عامل مقاومت به آموکسی سیلین و احتمالاً تتراسایکلین می باشند. بحث و نتیجه گیری: مروپنم، آمیکاسین، نیتروفورانتوئین، جنتامیسین و سیپروفلوکساسین برای درمـــــان اکثریت عفونت های مجاری ادراری مرتبط با E.coli مناسب هستند. پلاسمید 21 کیلو دالتونی ممکن است در گسترش مقاومت به آموکسی سیلین و تتراسایکلین دراین منطقه نقش داشته باشد.
Introduction: Assessment of antimicrobial resistance patterns and plasmid profile of 100 uropathogeic E. coli isolated from outpatient with UTI in Ilam(western Iran) was designed as the goal of our research .
Materials & Methods: Urinary samples isolated from symptomatic UTI cases were identified by routine methods. Antimicrobial susceptibility testing was performed by Kirby Bauer disc diffusion method according to CLSI guidelines. Plasmid extraction kit was used for plasmid extraction.
Findings: All the isolated samples proved sensitive to meropenem. The resistance rates of E.coli were as follows: 81% for amoxycillin, 64% for tetracycline,21% for ciprofloxacin, 16% for cefepime and ceftazidime, 15% for ceftriaxone, cotrimoxazole and tubramycin, 12% for gentamicin, 5% for nitrofurantoin, and 1% for amikacin. Plasmid profile investigations showed the isolated E.coli carried plasmid ranging from 21KD to 1/5KD in size. The number of plasmid in each of these isolates ranged from one to eight of different sizes. The E.coli isolates contained a common plasmid with a molecular weight of 21 KD which proved to enjoy resistance to, amoxicillin and possibilly tetracycline.
Discussion & Conclusion: There are doubts as to the role of tetracycline and amoxicillin for empiric therapy of UTI. Meropenem amikacin, nitrofurantoin, gentamicyn and ciprofloxacin are active against the great majority of UTI associated E.coli. 1KD R-plasmid reservoir may contribute to the spread of amoxicillin and probablly tetracycline resistance in our region.
