بررسی سرولوژیک توکسوپلاسموز در زنان باردار شهر بابل،شمال ایران، 1392-1391
Serological study of toxoplasmosis in pregnant women in Babol, northern Iran 2012-2013

مقدمه: توکسوپلاسموزیس یکی از شایع ترین بیماری های عفونی در دنیا است که در نتیجه عفونت با تک یاخته داخل سلولی به نام توکسوپلاسما گوندی ایجاد می شود. معمولا آلودگی به این انگل در بالغین بدون علامت است ولی کسب آن در دوران جنینی از مادرمبتلا بدون علامت منجر به عوارض جدی در جنین می گردد. این مطالعه با هدف تعیین میزان شیوع و بروز توکسوپلاسموز در زنان باردار شهرستان بابل ، شمال ایران انجام گرفت.
مواد و روشها: در این مطالعه توصیفی مقطعی در طی سالهای 1392-1391 ، 175 خانم باردار در تریمستر اول بارداری شرکت نمودند. از هر شرکت کننده 2 میلی لیتر خون گرفته و نمونه سرم جداسازی گردید.سپس،آنتی بادی برعلیه توکسوپلاسما با استفاده از روش الیزا اندازه گیری شد. اطلاعات دموگرافیک شرکت کنندگان و همینطور فاکتورهای خطر توسط پرسشنامه جمع آوری گردید. تعدادی از این افراد در تریمستر های بعدی حاملگی مراجعه نموده و ارزیابی آنتی بادی های ضد توکسوپلاسما تکرار گردید.
یافته ها: میانگین سن افراد برابر 9/5± 4/27 سال بود. در این میان، 106 نفر(6/60%) دارای آنتی بادی ضد توکسوپلاسما گوندی و تعداد64 مورد (6/36%) فاقد آنتی بادی اختصاصی بودند. میزان شیوع عفونت در ارتباط با سن و محل زندگی متفاوت بوده ولی این اختلاف از نظر آماری معنی دار نمی باشد (05/0 p>). همچنین، ارتباط معنی داری آماری بین میزان شیوع آلودگی و سایر فاکتورهای خطر مانند سابقه سقط و نحوه مصرف گوشت مشاهده نشد. هیچ موردی از بروز عفونت حاد توکسوپلاسما در مادر و یا توکسوپلاسموزیس مادرزادی مشاهده نشد.
نتیجه گیری: این مطالعه نشان داد که بیش از پنجاه درصد آلودگی با توکسو پلاسما در بابل در سنین کمتر از بیست و پنج سال رخ می دهد. ازطرفی با توجه به اینکه 6/36% از جمعیت مورد مطالعه فاقد آنتی بادی ضد توکسو پلاسما بودند ، انجام یک نوبت غربالگری در تریمستر اول بارداری ضرورت دارد تا با توجه به نتیجه آزمایش، اقدامات لازم جهت پیشگیری از توکسوپلاسموز مادرزادی، بعمل آید.
Introduction: Toxoplasmosis is one of the most common infectious diseases in the world and it caused by an intracellular protozoan called Toxoplasma gondii. This parasitic infection is usually asymptomatic in adults, but it become complicated if fetus infection occurred. This study aimed to determine the prevalence and incidence of toxoplasmosis in pregnant women in Babol, northern Iran.
Materials and Methods: In this cross-sectional study during the years 2012-2013, 175 pregnant women in the first trimester of their pregnancy participated. Two ml blood was obtained from each participant and serum was separated. Then, anti-Toxoplasma antibodies were measured using ELISA method. Participants' demographic information and the risk factors were collected by questionnaire. Anti-Toxoplasma antibodies were measured in the second and third trimesters of pregnancy for who attended to the study.
Results: The mean of age was 27.4 ± 9.5 years. 106 cases (60.6 %) had anti- Toxoplasma gondii antibodies (IgG) and 64 women (36.6 % ) had no specific antibody. The prevalence of infection was different in relation with age and location, but the difference is not statistically significant (p >0.05). Also, a statistically significant difference between prevalence of infection and other risk factors, such as abortion was not demonstrated. No case of acute Toxoplasma infection in the mother or congenital toxoplasmosis was reported.
Conclusion: This study showed that over fifty percent of Toxoplasma infection occurs in twenty-five years of age. Furthermore, it showed that 36.6% of the studied population (pregnant women) was seronegative. Therefore, Toxoplasma infection should be screened in pregnant women in the first trimester of pregnancy in order to perform preventative measure of congenital toxoplasmosis.
