شیوع باکتری های مقاوم جدا شده از مواد غذائی آشپزخانه بیمارستانی در تهران
Prevalence of Antibiotic Resistant Bacteria Isolated from Foodstuff in Kitchen of a Hospital in Tehran

مقدمه: بیماری های عفونی در بیمارستان ها یک مشکل عمده در جهان می باشد. برخی از این عفونت ها ناشی از مصرف غذاهای آلوده در بیمارستان است. هدف از این مطالعه بررسی خطر انتقال باکتری های حائز اهمیت از نظر بالینی، از غذاهای بیمارستانی به محیط بیمارستان بود.
مواد و روش ها: فرکانس پاتوژن های باکتریایی مهم مسئول در عفونت های بیمارستانی در میان نمونه های مواد غذایی، کارکنان و ابزار به کار رفته در بخش طبخ بیمارستان مورد مطالعه قرار گرفتند. کشت با سواب و سنجش تعداد پلیت های هوازی برای جداسازی باکتری های مورد جستجو به کار رفت. شمارش کلنی و تعیین هویت مولکولی و بیوشیمیایی بر اساس روش های استاندارد تخمین زده شد، حساسیت ضد میکروبی هر جدایه بر طبق آخرین راهنمای استاندارد آزمایشگاهی بالینی برآورد شدند.
یافته های پژوهش: از میان 200 نمونه تحت مطالعه بالاترین آلودگی باکتریایی درمیان نمونه های ادوات طبخ دیده شد.(40 درصد) هم چنین از میان گونه های باکتریایی جداسازی شده آلودگی به گونه های استافیلوکوکوس اورئوس در بالاترین میزان تعیین گردید،(16 درصد) در حالی که آلودگی به اشرشیاکلی 8 درصد بود. سایر پاتوژن های مسئول عفونت بیمارستانی در این نمونه ها از جمله سودوموناس، اسینتوباکتر و انتروکوکوس فراوانی کمتری نشان دادند.(6/4 درصد) از میان جدایه های استافیلوکوکوس اورئوس، 7/18 درصد آن ها الگوی مقاومت دارویی MDR-MRSA را نشان دادند. این الگو در 9/52 درصد از جدایه های اشرشیاکلی مشاهده گردید اما در سایر جدایه های باکتریایی دیده نشد. هم چنین نمونه های ادوات طبخ بالاترین میزان فراوانی به عوامل باکتریایی دارای الگوهای مقاومت دارویی چندگانه را مشخص نمودند.
بحث و نتیجه گیری: شیوع بیماری های ناشی از غذا در بیمارستان ها گزارش گردیده است. فراوانی بالای باکتری های حائز اهمیت از نظر بالینی در میان ظروف در مقایسه با نمونه های مواد غذایی و کارکنان بخش طبخ و حضور باکتری های روده ای و باکتری های نشانگر پوست در این نمونه ها، نقش احتمالی غذاهای بیمارستانی را به عنوان یک عامل خطر برای انتشار باکتری های بیماری زا در محیط بیمارستان نشان دادند.
Introduction: Infectious diseases among ho-spitals are a major healthcare problem in the world. Some of these infections are cau-sed by ingestion of contaminated foods in the hospitals. The aim of this study was to investigate the role of hospital foods in the transmission of clinically important bacteria into the hospital environment.
Materials & Methods: Frequency of the main bacterial pathogens responsible for h-ospital infections were studied among sam-ples from foods, food handlers and food p-rocessing devices. Swab culture and aerobic plate count assay were used for the isolation of suspected bacteria. Colony count of each culture and biochemical or molecular ident-ification of each isolate was determined according to standard methods. Antim-icrobial susceptibility of each bacterial iso-late was determined according to the latest clinical laboratory standard guideline.
Findings: Among the 200 samples under study, the highest frequency of contam-ination (40%) was seen among utensils. Furthermore, Staphylococcus aureus show-ed the highest frequency (16%) among the obtained isolates, while Escherichia coli re-presented 8% of the contaminations. Fewer rates of contaminations were belonging to other responsible bacterial pathogens, inclu-ding Pseudomonas spp., Acinetbacter spp. and Enterococcus spp. (0-4.6%). Among the Staphylococcus aureus isolates, the mu-ltidrug resistance phenotypes were observed to be 18.7%. This phenotype was observed among 52.9% of Escherichia coli isolates but not among other isolates. Among the studied samples, the highest frequency of multidrug resistance was seen among uten-sils.
Discussion & Conclusion: Outbreaks of food borne disease has been reported in ho-spitals. High frequency of clinically imp-ortant bacteria among utensils compared with samples of foods or food staffs, and the presence of enteric or skin marker bac-teria in these samples revealed the possible role of hospital foods as a risk factor for dissemination of pathogenic bacteria into hospital environment.
