بررسی عوامل باکتریال عفونت های ادراری و تعیین مقاومت آنتی بیوتیکی آن ها در بالغین بستری و مراجعه کننده به بیمارستان فرشچیان شهر همدان

مقدمه: مقاومت روز افزون باکتری ها به عوامل ضدمیکروبی مشکل عمده در سراسر جهان می باشد. لذا اطلاع از وضعیت سویه های ایجاد کننده عفونت ادراری در برابر آنتی بیوتیک های رایج در درمان از اهمیت خاصی برخوردار است. این مطالعه با هدف بررسی عوامل باکتریال عفونت های ادراری و تعیین مقاومت آنتی بیوتیکی آن ها در بالغین مراجعه کننده و بستری در بیمارستان فرشچیان شهر همدان در سال 1390انجام شده است.
مواد و روش ها: این مطالعه توصیفی-مقطعی بر روی 214 فرد مراجعه کننده و بستری شده در بیمارستان فرشچیان شهر همدان در سال 1390انجام شد. بررسی مرفولوژی و شناسایی باکتری های ایزوله شده با استفاده از رنگ آمیزی گرم و بکارگیری آزمایش های بیوشیمیایی افتراقی انجام شد. آزمایش حساسیت آنتی بیوتیکی با دیسک های آنتی بیوتیکی کوتریموکسازول، نالیدیکسیک اسید، جنتامایسین، سفتریاکسون، نیتروفورانتوئین، توبرامایسین، آموکسی سیلین، افلوکساسین و سپیروفلوکسازین با استفاده از روش کربی-بائر انجام گرفت.
یافته های پژوهش: از 141بیمار مورد مطالعه، 2/54 درصد زن و 7/45 درصد مرد بودند. شایع ترین عوامل ایجاد عفونت به ترتیب شامل اشرشیا کلی(61 درصد)، گونه های انتروباکتر(7/10 درصد)، استافیلوکوک ساپروفیتیکوس(5/8 درصد)، پسودوموناس آئروژینوزا(9/7 درصد) و گونه های پروتئوس(1/7 درصد) بودند. در مجموع بدون در نظر گرفتن نوع میکروب، بیشترین مقاومت به آموکسی سیلین(3/81 درصد در جنس مونث و 70 درصد در جنس مذکر) و کمترین مقاومت به نالیدیکسیک اسید(11 درصد در جنس مونث و 16 درصد در جنس مذکر) بود. اشرشیاکلی به عنوان شایع ترین عامل ایجاد کننده عفونت ادراری همین الگو را در هر دو جنس مذکر و مونث نشان داد.
بحث و نتیجه گیری: بر اساس نتایج این مطالعه، شایع ترین عامل مولد عفونت ادراری در بالغین اشرشیاکلی بوده و سویه ها بیشترین مقاومت را نسبت به آموکسی سیلین و کمترین مقاومت را نسبت به نالیدیکسیک اسید نشان دادند.
Introduction: Increasing resistance to anti-microbial agents in bacteria is a major problem around the world. So, our kno-wledge about the bacterial strains status causing urinary tract infection and their resistance against common antibiotics is an important issue in treatment. The aim of this study was to indentify the bacteria causing urinary tract infections and to determine their antibiotic resistance in ad-ults hospitalized and referred to the Farshc-hian hospital in Hamadan in 2011.
Materials & Methods: This descriptive cross-sectional study was performed on 214 individuals hospitalized in or referred to the Farshchian hospital in 2011. Morphological evaluation and identification of the isolated bacteria were performed by Gram staining and biochemical tests. Antibiotic suscept-ibility tests were done with cotrimoxazole, nalidixic acid, gentamicin, ceftriaxone, nitr-ofurantoin, tobramycin, amoxicillin, oflo-xacin and ciprofloxacin disks and the Kirby-Bauer method.
Findings: Of 141 patients, 54.2% and 45.7% were female and male, respectively. The most common causes of infection were Escherichia coli (%61), Enterobacter spp. (%10.7), staphylococcus saprophyticus (%8.5), Pesudomonas aeruginosa (%7.9) and Proteus spp.(%7.1). In total, regardless of the type of bacteria, the highest resis-tance was against amoxicillin and the low-est resistance was against nalidixic acid. Escherichia coli as the most common cause of urinary tract infection and had the same pattern of infection in both sexes.
Discussion & Conclusion: According to our study, E. coli was the most common cause of urinary tract infection and the strain had the highest and lowest resistances against amoxicillin and nalidixic acid, respectively.
